Overview

Could you lift the spirits of a lonely person in hospital? Do you have the skills to support and encourage them to engage with community services to reduce the likelihood of hospital readmission?

Responsibilities

In becoming a Hospital Liaison Mentor you will work closely with our Hospital Liaison team to ensure existing and prospective users of our service receive support during their admission and immediately following discharge.
 
You will befriend individuals who may have no other visitors during their inpatient stay and who may be frequent attenders at hospital. You will explore with them underlying barriers to their engagement with community-based services, and liaise with the team to ensure any unmet needs are highlighted and can be addressed. 
 
In some cases you will follow up these Mentees post-discharge with short-term practical support to assist them to engage with services to enable them to thrive in their community.
 
If you have a few hours available each week, are friendly and outgoing, understand the stigma associated with substance use, and would like to be an advocate for recovery we want to hear from you.
 
All our volunteers are given opportunities to build on their existing knowledge and skills through further training, and ongoing support and supervision from a designated Volunteer Co-ordinator.
